Supervisor of Employment and Transition Services
CHOICE Services
The Supervisor of Employment and Transition Services is responsible for the overall delivery and coordination of Employment and Transition Services for the CHOICE program.
ESSENTIAL FUNCTIONS
The essential functions of this position consist of the ability to write, read and comprehend what is read, and perform math skills. Computer skills, advanced expressive and receptive language skills are required in the execution of the position. Must possess a valid driver’s license, be able to transport and be able to be physically present at each service location. The performance of crisis intervention skills are required as necessary. GENERAL WORK PRINCIPLES

D. Act as an appropriate role model and present an acceptable appearance and image when representing the agency.
E. Exercise common sense and good judgment in all decision making and contact supervisory staff for direction when necessary and required.
F. Have full knowledge of Integration, Everyday Lives, Hope Enterprises, Inc. Policy and Procedures and current ODP and OVR practices and practices and philosophies.
G. Identify needs and weaknesses of service delivery. Participate in the development of policies and procedures to improve operation of service delivery.
H. Promote a trusting relationship and remain professional when working with individuals/families.
I. Facilitate and maintain relationships between the Administrative Entity, Supports Coordination Organization, day programs, and other external support services.
J. Avoid allowing personal beliefs and values to influence dealings with individuals receiving services.
SUPERVISION
A. Supervise and provide leadership to Direct Support Professionals.
B. Participate in orientation and ongoing training for staff.
C. Ensure compliance with Integration, Everyday Lives, Hope Enterprises, Inc. Policy and Procedures, and current ODP practices and philosophies.
D. Ensure Association of Community Rehabilitation Educators (ACRE) and Community Participation Support (CPS) training is completed within established timelines.
E. Assist in interviewing and hiring of staff.
F. Complete and discuss performance evaluations.
G. Address performance issues and follow up under the direction of the supervisor and ensure consistent application of disciplinary actions in conjunction with agency policies.
H. Review, approve and submit time records, expense vouchers, and other required personnel forms.
I. Ensure required operational paperwork is reviewed, approved, and submitted in a timely manner.
J. Develop work schedules for staff. Coordinate temporary work assignments to alleviate staff shortages and assure consistency in providing services.
K. Develop caseloads to ensure consistency in service delivery.
L. Meet regularly with staff.
M. Ensure attendance at required meetings and trainings. N. Ensure trainings are complete by the required timeframe. O. Notify and consult with supervisor when additional support is needed.
RESPONSIBILITIES
A. Obtain Association of Community Rehabilitation Educators (ACRE) and Community Participation Supports (CPS) training certificates.
B. Assist with increasing employment and transition opportunities through regular contact with employment businesses, Office of Vocational Rehabilitation (OVR) and school districts.
C. Attend transition councils at the local school districts.
D. Ensure the use of technology to support service delivery.
E. Provide service delivery.
F. Maintain and monitor individual’s records for compliance.
G. Be familiar with budgets, ensure expenditures are consistent with budget limits, and make recommendation for budget changes as necessary. Be familiar with Individual Support Plan’s (ISP).Ensure DSP’s are trained on the ISP to achieve individuals’ outcomes and implementation of services including the CWDS system and ODP service delivery.
H. Monitor service delivery and unit utilization. Communicate utilization to DSPs as necessary.
I. Ensure accurate and timely submission of units and invoices to appropriate billing departments.
J. Have full knowledge and comply with Electronic Visit Verification (EVV).
K. Have full knowledge of Office of Vocational Rehabilitation (OVR) policy and procedures and the OVR’s electronic system
L. Complete and submit monthly progress notes within required timelines.
M. Assist with maintaining agency vehicles as required. Includes maintenance, cleanliness and documentation at least one time per month.
SERVICE DELIVERY
A. Participate in the referral process, as necessary.
B. Ensure participation at transition council and ISP/Team Meetings
C. Assist DSP’s to ensure individuals progress with outcomes and recommend changes as needed.
D. Provide routine contacts to maintain positive relationships with existing employment connections.
E. Facilitate, assist, or represent the individual when there are barriers with service delivery.
F. Ensure provision of services to OVR clients based on the timelines set forth by Supported Employment and/or PreETS guidelines.
NOTIFICATION OF INCIDENTS
A. Have full knowledge of and comply with Incident Management Policy.
OTHER DUTIES
A. Make presentations to other agencies, organizations, and interested parties.
B. Participate in meetings/committees as assigned.
C. Attend and participate in appropriate training sessions, seminars, and/or conferences.
D. Perform other duties as may be required or assigned.
HOURS
- Exempt position; based on 40 hours weekly, evenings and weekends may be required.

QUALIFICATIONS
- High School diploma or GED and 4 years of working directly with persons with disabilities
Preferred – Associate degree or 60 credit hours in Human Services or a related field, and 4 years of working directly with persons with disabilities
